School of Origin vs. School of choice.

If have a situation where a Kindergarten student who attended pre school in our school district last year became homeless, but the Pre-School does not have district lines, they do not have any school of choice as it is open to anyone. Just before school started their Kindergarten year. The enrollment paperwork had a District address, but before school actually started, they had to move out of the school district.

If the pre-school is affiliated with our district, but is technically its own entity, would that parent now need to fill out a School of Choice Form or would the student be considered school of Origin. If they are School of Origin but move during the school year, do they have to fill out school of choice forms.

I know this is technically two separate questions, but they both relate to school of choice.
